Going Home

We want to ensure that you have the information and support you need to continue your recovery at home.

Preparing For Discharge

Either your attending physician, a hospital staff member who is helping care for you, or your nurse will tell you when you can expect to be discharged. Then you can make arrangements to go home. Your attending physician will write your discharge order.

Checkout Time

Checkout time is 11:00 am on the day of your discharge.

Pharmacy

You may have the Dayton Heart Hospital Pharmacy fill any prescriptions you receive when you are discharged. Your nurse can arrange for this service. If you prefer to have your prescriptions filled by your own pharmacy, ask the nurse for the written prescription orders.

Dayton Heart Hospital accepts most prescription insurance plans. Some insurance policies will not reimburse medication costs, so be sure to check with your insurance provider about your coverage. If you have a prescription card plan, you will be responsible for paying the co-payment. The co-payment cannot be included in the hospital bill.

Taking Charge Of Your Health

During and after your stay at Dayton Heart Hospital, our staff offers a range of services to help you take charge of your health and adopt a healthier lifestyle. Services include nutrition counseling, which can focus on specific diseases; weight management services; fitness consultations and classes; stress management workshops; and smoking cessation programs.

Rehabilitation or Nursing Home Facilities

Should you need a referral to a rehabilitation or nursing home facility, case management staff will assist you and your family to make those arrangements.

Home Health Care

If you need home health care, your nurse care coordinator or case manager will help arrange services. It is your right to choose any home health care provider you wish.

Gratuities

Our staff does not accept gratuities. It is our pleasure to care for you.

Patient Survey

We want to know what you think about the care you received at Dayton Heart Hospital. Your candid opinion helps us serve our patients better. Shortly after you return home, you will receive a survey in the mail. Please take the time to fill it out completely and return it in the postage-paid envelope we provide.
